To obtain a copy of a "previously approved" application file and list of sources, as a reference, please follow these instructions:
Step 1: Click here to access the purchase order, and use PayPal or Credit card to pay the $5 fee.
Step 2: Send an email to FirstFamilies@mosga.org and indicate the name of the Ancestor and type of Certificate awarded.
If you need to send the request by postal mail, include a check for $5.00 payable to MoSGA:
